1. Print out the simple application, complete and sign:

- Three months previous processing or bank statements (if available),
You can complete your application using the same name on your bank account. The patient’s credit card statements will show the following- “CASH-POS Transaction”.
3. Mail the completed application and supporting documentation:
- a voided check or bank letter showing a banking relationship (where funds will be deposited),
- a clear copy of the owner’s driver license or passport.
